Kill Raphael's Old Enemy Quest Bugged

I've killed the Orthon and Raphael won't trigger. I had Astarion in my party when I convinced him to kill himself and the quest is still saying "we encountered" instead of that he's dead. HEELP!

Did you pick up the Orb and Exit the dungeon? Raffy should meet you there.. unless they changed it, I noticed in my playthrough raffy appears in the throne room if you help the orthon and kill the last justiciar.
