Non invasive procedure?

Hi, can anyone help me how to get this achievement?
i did everything including :
- Nurses kill each other
- Convinced the doctor to be operated
- Killing him in 1 turn

i read online that some people said it's through dialogues, then some people also said it's through combat but i did both and still no achievement

Also the same with Toll Collector achievement related, did both in combat or through dialogue no achievement

If anyone can help me with some answer, big thanks

do you have mods installed without the modded achievement enabler thing

you need to kill Malus in combat without him performing surgery on you; you can't get this one through dialogue

for the Toll Collector, you can get it through dialogue I believe. i'm not exactly sure.

but make sure you don't have mods like TaKo says

yeah i didn't have that, i didn't thought it will locked me out from getting achievement when i only install aesthetic mods. i will try installing that

well i did all, through combat even making him unable to move till his death
through dialogue
but maybe it's because of the mods i will try that
