Stuck with 'open the door to inner sanctum'

Hi guys!
I'm stuck with this quest in the Gantlet of Shar and I didn't know what I did something wrong.

Have all 4 gemstone. One, I placed the alter transportation.
and the others rest 3 gem I keep it to place on the last alter in front of inner of sanctum.
But when I did placed 3 gems. it'll show as I place just only one gem.

And nothing happen after this. I can't open the door.
Anyone has any idea to fix it?

This has appeared several times recently and probably a bug or mod issue. The ability to add all three at once was introduced recently, so the old method of entering turn-based mode to complete may no longer be valid. What I can offer is a spoiler that last I knew was not altered.

The inner door can be opened with a knock spell or scroll

Can confirm it worked up to U6 release.

a) mods related issue

b) use knock spell to bypass it - as in: respec / get a hireling wizard -> skill Knock -> Knocking door -> move on
