Multiplayer and Romance Question
I’m trying to look up this answer on google but can’t seem to find anyone talking about it and I’m genuinely curious! If your multiplayer campaign consists of 4 irl people and not companions (like Karlach, Wyll, Gale, etc.) Are you still able to romance companions even if they’re not apart of your main party?

I don't think you could make that work, unfortunately.
The romances require a certain amount of approval that would be very hard to get without ever having a character in the active party- they're not around to approve/disapprove of your choices- and I think there's a few places where you'd just be unable to do what you need to without bringing someone out of camp.
e.g. Karlach's quest requires bringing her to talk to a specific NPC who isn't in your camp, and I don't think her romance can work without doing that.

The last person asking this exact question a few days back was ... surprise ... ME.
And the answer is, no. No, you can't romance other players, you can't even romance companions under the other player's control, you have to have them in your own party to being able to romance them. But not the actual player character. Even if they disconnect, the only option you're left with, is dismissing them to your camp.

About companions, yea. You can romance companions stationed in the camp.

What about a party size mod?

What about it? It doesn't affect romances in any way. It gives you more slots for your party and the parties of other players helping you on your (and their) quest. The mod works, it expands the possible max number of players to 8 standalone, or less with some companions. Again, no effect with romances, you have to keep companions in your party to be able to romance them (or leave them in camp, but they won't accumulate approval points much that way).

On a 4p run I managed to romance Laezel ( broke up with her cause of minors though ).

Didn’t play all the game with 4 players actively, but when we played 2 or 3 players the other was waiting in camp ( no companion in party ).

Or Maybe I took her to question the tiefling and to talk at the creed, but I can’t tell for sure ( too many runs ).

I don't follow. With more slots, can't some of them be assigned to companions?

Yes, they can, the mod enables 8 slots for players + additional slots for companions (can't say how many exactly). It behaves like in vanilla game, you don't assign slots manually. But if you have less than 8 players, the more slots you get for companions. To have them assigned under other player's control, you go to the "Multiplayer" menu and click arrow next to a companion's portrait to move them to the other player's party. But then the companion will be romanceable by that player, not by you. If you want to romance companions, keep them in your party, don't assign them under other players. Hope everything is clearer now.
