Can you share a savegame it happens on.
It's a quick check to see if it has mod dependencies, which then become your suspect.
In the future just to be safe, if you go to the main menu in front of an elevator and then load back into your savegame, there should be no chance that the elevator will bug out. Annoying, I know, but it's better than losing an honour mode playthrough to nonsense.
Just did the Myrkul fight last night in honor mode and rode that elevator, never seen an issue with it. Not sure how you fell off as that elevator is huge. I have like 30 mods loaded but had the standard 4 in the party.
Did you have a mod with more than 4 in the party? If so, you need to split the party and manually move everyone to the center area of elevators as they tend to spread out towards the edges.
I've been playing with a party of 7/8 and having zero issues with elevators. The devs have made using oversize parties flawless in my current playthrough.
When did you start your current campaign?
Happent to me on my recent playthrough as well, Astarion just fell down did not follow the lift.

Also, unfortunately there's no telling when (and if) these bugs will be fixed. Devs are preoccupied with wobbly scrotums and kissing animations, they got no time to fix basic stuff. Sometimes they do - but that has also proven to be error-prone and while fixing one thing they tend to break two others, so there's that too.
To be fair, the game has less issues than many of the similar ones, but that's the bar too low I guess. So when you play it on HM - game mechanics and the combat are probably not the thing that will end your run. Some glitched endlessly looping dialogue / a buggy elevator or a classic corrupt save are far more likely candidates for that.
a) I use Party Limit Begone, and often have parties of 6 - all of them fit on the elevators, although if it's a cutscene it usually just shows 4
b) I've had several playthroughs of this game now
and
c) I've never seen THIS elevator bug - though BTW I HAVE seen PLENTY of others (just not ones that would break the game or cause a party wipe) (and yes I use mods but I know the ones I've seen which I can rule out being mod caused)
On the other hand, I certainly can count on all my fingers the people who have said they HAVE fallen through elevators.
So here comes my conclusion: if it isn't affecting everybody, but only affecting some people, and maybe even a small number of people, other causalities need to be considered. Then that "this is a bug in the game that affects everybody" as it clearly doesn't. So then what could be the cause?
a) mods, of course, though folks SAY they are running none but have run into this
b) I get this weird sense it happens more often in multiplayer than SP. Might be wrong, but if so, maybe there's a reason for that.
c) hmmmm ... how long the game has been running ... is this some kind of memory leak?
d) improper or broken installs of files for the game
e) hardware issues
You will not get much help on this forum: Many are in this weird war where one side is just trying to dismiss and bury any issues people are having with the game and act like it is flawless and the other side is just trolling for points.
Those who genuinely try help can only advise you to try delete the game's cache files or reinstall the game entirely but as this issue has been around even after multiple reinstalls it is best to just go to Larian themselves and submit a bug report and open a support ticket.

Actually .. this "appearing on roofs" bug I have seen ... but it seemed to me like it was being caused by Party Limit Begone. Like I would fast travel to a location with a party of 6, and 5 would be in the building, and 1 would be up on the roof.
Funny story: one time I tried jumping off one of those roofs, figured I would take survivable damage ... well, I survived but I landed on a passerby, killing them, and suddenly Flaming Fist were after me like white on rice. I was like "hello? I didn't try and kill them INTENTIONALLY" but FF don't strike me as the brightest law enforcers.
Anyway, I usually always make sure to have Feather Fall or Fly scrolls available, solves this issue.
I just lost 4 hours cause of random crash at Moonrise Towers, not to mention lots of other issues on my HM playthrough. As much as I see why people love this game, it could use few more patches thats for sure, cause stuff like that is extremely depressing. When I started my HM run I didn't expect THAT many issues I've encountered. Not after years of early access and half a year after the actual release of the game.
