What happens if you take a character different from your romance to Zethino in Act 3??
It modify something into the relationship with the character you're having a romance with??

I'm into a romance with Karlach, but it seems that I can take Laez'el and Shadowhart too to Zethino.

You can take anyone with a high approval level. A mid level approval character will refuse politely, a low level approval character will give some variation of "Gross."

So, for the romance you're in, it change nothing if you choose someone you've doing the romance or another character??

So far as I know it changes nothing. After all even with magical visions involved it is ultimately just a carnival game.
I was slightly wrong tho; Jaheira and Minsc will always refuse, though again more politely if you're high on approval.

The Love Trials don't impact approval or romance status. Nope. As said, it's just a carnival game. The prize is learning more about your beloved. (Reactions will vary based on what you get right or wrong, but that's it. If you need to get all 3 questions right, it's all spoiled over at bg3.wiki.)

There's a reveal on who Zethino really is at the end of the Trial of Love.

To say more is spoilers, and again, debatable whether it is "worth" finding out.

there's a minimum of two times she can appear; if you trigger them before talking to the dryad you get the real Zethino
