Our Fiery Friend (spoilers, I guess)
Was I supposed to do something about Karlach's heart in the Steel Foundry? I seem to recall some dialogue hinting at that, but I just got finished blowing the place up (after rescuing the Gondians and their families), and I brought Karlach with me, and there was no mention of her heart.

In the end, whatever you do, Karlach has to return to Avernus or die so... No matter. But I think not, anyway. I didn't, at least.
