Should there be a “betray Bhaal” ending?
I want to accept his gift, but I want to destroy the nether brain. Should there be an needing for this where I can have a happy ending without turning into some zombie?

The only happy ending is if you beat Orin and reject Bhaal. As it should be.

Now you could argue that rejecting Bhaal and dominating the brain are a happy ending, too. But that thing seems to be too smart and powerful so it always looked to me as a risky proposition at best and insane one, at worst.

But I need to accept Bhaal. If I reject Bhaal Minthara breaks up with me. So my only option is to accept my inheritance.

So my question is can I accept Bhaal’s gift but when the time comes to destroy the Brian, destroy it instead of control it? What happens if I do that?

I mean I just defeated the nether brain, something more powerful than Bhaal self. So what can he do to me?

Netherbrain isn't more powerfull than Bhaal, not by a long shot. While forbidden from direct interventions by AO, he's still a god. Along the things he can do, is kill you on the spot if you betray him, netherbrain or nah.

So why not add a quest to kill Bhaal and regain your freedom? Or take his place? We are strong at level 12 no?

No. Max level in DnD is 20, and for the most part, gods don't even have a stat sheet. They are considered so out of reach you would never be given the chance to challenge their spot without being a god yourself. The only one with a shot at bhaal is god-Gale.
