I have a similar machine. MacBook Pro M2 Max 16", 32GB RAM, 30 core GPU.
Performance in the world around those spots is worse than it should be, too.
Must be some specific shader or something.
Did you try this?

I tried, but it didn't work.
Then at last resort there is only the version rollback to check whether the issue is drivers related or indeed game related. Anyone taken that route yet?
Even if everybody has the same driver issues, it's still an issue with drivers everyone has. In which case it's still a device testing issue and can only be fixed by Larian.
I also tried deleting the caches, no effect. Won't try rollback, I like my save games and hope for a fix - it's not unplayable, just really annoying.
Make sure you all at least send in your issue with details of your Macs here
via email support@larian.com
or
via online form https://larian.com/support/baldur-s-gate-3#modal
only then it gets tracked and investigated by Larian Studios properly
Hopefully Larian can figure this one out quick!
Using the rollback guide of mine is really not that kind of a big deal.
Once you understand the basics, it is a matter of minutes. Plus you have always the option to "freeze" a game version prevent updates. That is how Windows users keep playing with their Mac friends whenever the versions do not match up.
Could you please share a link to your rollback guide?
Btw, tried to install it on another mac with M2 and there's exactly the same issue: a massive FPS drop during dialogs.
