What to do with the Kuo-toa (minor spoilers)

I really am unsure of what the right thing to do here is,
If I leave them alive and they worship me, they say they’re going to keep doing atrocities in my name.
If I kill them though, I’m afraid of missing out on content.

What should I do?

Unless you want a permanent attack buff against bleeding targets (by sacrificing a companion) , there isn't really any long term impacts of your choice.

I think I ran through the dialogues twice to figure out what outcome I wanted. The first time I tried killing the imposter leader and leaving the rest alive, which felt a little iffy based on their other dialogue, like you said. The second time I ended up triggering combat with everyone and wiping them out (I can't remember the exact choices I made).

The whole thing felt like more of a humorous secret area than anything impactful.
