What actually determines whether or not an enemy is set on fire?
Can my Fire Bolt set enemies on fire? How about a Fireball or Burning Hands? How do I know what can or can't set enemies on fire?

The game is based off of D&D 5e and most of the fire spells do not set things on fire Rules as written(RaW). They will in this game if you have greased the surface that the target is standing on. It's better to assume not most of the time. Wall of flames does ignite things though. This is mainly because players wouldn't use Fireball if it did because it would destroy loot with a stingy DM and so they specifically worded a lot of spells so that they didn't have to worry about that.

That's the thing, I'm pretty sure enemies HAVE been set on fire by some of these spells without any flammable surfaces underneath them, just not all the time.

Like I said, it's better to assume not. Some spells do, most don't. Grease or grease like effects will trump what the spell does as well as any magic items that may add effects to your spells.

Maybe they were standing on flammable surfaces and I just didn't notice.
