That is manageable at level 12.
Aid upcasted gives 25. Heroes feast another 12 (I think). Throw in the Tough feat for another 24.
CON 16 on a Barbarian means she gets 10 HP a level with 15 HP at level 1. At level 12, that's already 125.
Aid at 6th level adds 25 to your Max HP, 125 goes to 150.
Heroes' Feast ups that another 12. 162.
Say she has Tough, that's +24 for 186.
Now add that CON Amulet from House of Hope for another +36, and she can theoretically reach a Max HP total of 222 on Balanced and higher difficulty.
You can apply the same logic to any character, we'll use Gale since WIZ and d6 hit die, while also being able to hit 16 CON easy with his base stat array.
He can hit 183 Max HP with the same setup.

Realistically speaking . . . you don't need that much HP. My Monk who has an average of 87 Max HP at level 12 goes straight to 124 Max HP from Aid + Heroes' Feast, but even then it's more than passable since high DEX/WIS save prof. (I took Resilient - WIS since her DEX was maxed), Evasion, AC 21, and mobility from hell.
