Beyond that, you hit basically the same story beats as you would with her as a companion, but you have more complete agency over how things play out as opposed to needing to convince Lae'zel as someone else.
There's not a lot of differences that it feels like an altogether different playthrough compared to playing as a Githyanki Tav, but it's a neat feature that they let you do if you want to. And with Withers you can respec her if you prefer playing as a different class like Wizard or Cleric. (You can also do this when playing normally as well you don't need to be playing as an origin to respec them).
Different companions have different dialogue appear. Like Astarion has a different scene for every companion if you bite one of them for the first time. Karlach has a scene if you play as her and meet Wyll for the first time, and vice versa. Etc. The companion that probably has the most differences is Gale since his cat becomes a character you can interact with in Act 3.
I wanna see Astarion biting Karlach. I fear he burns to ashes...
(In my playthrough I did not get bitten by Astarion. He told me he is a vampire and never tried anything.)
How are the personal quests? Like Karlach engine issue if we play her? Or playing Laezel, does she still tell we need to find the creche as fast as possible?
Heartbreaking. Playing as Karlach creates probably the most heartbreaking version of Act 3 companion events. You get to see unexpected, maybe even rare, cutscenes. Which are going beyond what you know from all the other characters.
I've never really been a fan of the "making a character that you can pretend is you" deal that comes with custom character creation, so having a lineup of interesting characters to choose from with the Origins was great.
Playing as Shadowheart, being centrally tied to the main plot by having the objective "protect the artifact" which consistently came up during Act 1, and being tied closely to the Shar curse in Act 2, it felt like my character actually belonged to the plot. In contrast to a Tav playthrough where you're kind of just plopped in there and expected to fill in the gaps yourself.
