There is only 1 mod that essential for me "Transmog Enhanced". You can swap your equipment visual with another while still keeping all the stat intact. Gone are the days where i have to look like a clown to become powerful.

I would say use "Recruit Any Npc" mod.

This mod Allows you to befriend any NPC, and or "Clone" (Make another custom character without having to multibox), and eliminated the need for "party Limit be gone", as you're able to recruit as many companions as you'd like in your party at once.

Just alone having the ability to create more custom characters, and mess around with builds/classes/races without having to restart the game, and starting anew alone is worth it. It's allowed me to try out so many diff classes/builds, WHILE being able to keep those characters in reserve, should I so desire to recruit them back. And yes, they scale with your levels as well, so they don't just start at level 1.

It's a pretty badass mod.

Definitely Vanilla Hair Mixer, it allows you to select more than one hair for your tav in the character creator and makes for more fun hairstyles

I'd also recommend KV Camp Events which adds a little exclamation point over your head every time you have a night nighttime event so that you never miss any of them

The only mod that stands out for me is Combat Randomizer. If you want a skilled challenge.

My personal faves: (there are many at Nexus that require script extender and look tasty, but I can't use those)

General --
Party Limit Begone - use the scripting version, if you can, I would if I could
Unlock Level Curve
5e Spells Addon (contains 5e source spells Larian didn't use)
Improved UI
Additional 5e Feats mod (again, 5e feats Larian didn't use)
Secret Scrolls (adds scrolls for spells Larian didn't implement in the game)

Race Mods --
Aasimar
Half Dragon

Class Mods --
Artificer
Mystic (psionic class - fits this game's theme very well)
Vanguard
Drakewarden (ranger)
Pirate (rogue)
Hexblade (warlock)

Equipment Mods --
Useful Music Instruments (puts in the game musical instruments with actual magical properties, something bards should have anyway)
Drow Equipment Pack (adds some interesting drow flavored gear to the game, nice for Minthara or the drow hireling ranger though many can be used by non drow)

Unlike in most other games I like (there are not many) I don't use any mods which change game mechanics or combat, just visual mods.

- Basket Full of Equipment SFW version (I change all stuff I use to vanilla stats, like the sexy robe for my female Sorlock main which I made light armor with 13 AC)
- Native Camera Tweaks (to change the terrible default camera angles)
- WASD Movement mod (for more immersion, as the following mods)
- Better Hotbar 2
- Better Scaled Map
- Better Combat Log
- Dynamic Sidebar
- Less Annoying Hex
- Hip Sheaths and Visible Shields
- Shadowheart Keeps Black Hair

Cosmetics + QoL + gameplay + balance:

--> Cosmetic mods to go by to make Tav unique (hair, heads, body, you name it)
-- Vanilla weapons overhaul
-- Vanilla armor overhaul
-- Soul smithing
-- Sussur crafting extended
-- 5e spells
-- Immersive UI
--> submodules to go by (hiding the main panel or portraits, etc)
-- Lethal AI
-- Extra feats (5e ones)
-- Levels 13..20 curve (so that to level up past the start of Act 3)

I love the Trade with Withers and loot autoseller mods. It's so convenient to have junk directly converted to gold and sell the rest at camp to Withers.

Otherwise, I like 5e spells, unlock level curve 13-20, feats extended and the extra subclasses for rogues, rangers and especially clerics. (Twilight domain for Shadowheart for RP purposes anytime XD) And some absurdly op equipment mods to live my Dark Urge power fantasy now on my second full run. XD

I don't care much for aesthetics, so no hairstyles or extra camp clothes for me.

Somebody already mentioned Basket Full of Equipment SFW version.

I recommend this as well, but the NSFW version. Having your ragtag group of adventurers run around as a BDSM Dom squad is pretty great. Besides, nothing beats fighting a dragon while wearing a thong.

Oath of Conquest paladin (actually already in the game with some gith NPCs)
Undeath Warlock (actually already in the game with 1 gith NPC)
Abberrant Mind sorcerer
Psy Knight fighter (works very well for Laazel)
