Report a translation problem
Since the last hotfix, when i grab a needed quest item, the game doesn't recognize it, as if i don't have it. (Sacred idol and bitter divorce for example). When i disable mods, it works properly. Strange
There have now been many reports of the game not recognizing quest items in inventory. Likely due to the change made to allow quest items to be accessed from the Travelers Chest while in the field.
Probably going to have to wait for a Hotfix on this one. :(

It should as this is not a data issue but a code issue. That is my guess on it.
So it must be the mods, but I can't deactivate them. It makes my game crash.
I'll try deactivating Mod Fixer only.
Your character a random person, not tasked by the king, showing up out of the blue with the head of someone he wants dead. Good on ya!

Why would they? You voluntarily did that yourself.
So a code issue, but likely a deliberate code issue
Had the same problem with Nere's head quest.
I'm using the party limit mod (manual install where you just copy/paste the /mod folder, not using mod fixer), so I dismissed everyone to camp, then removed the mod and I was able to complete the quest.
I then went to Nexus where I got the mod, and lo and behold, the mod creator has released a new version for hotfix 16.

I installed the new version and reloaded the game at the point before I dismissed everyone, and I could now complete the quest WITH the mod active.
My suggestion is to check any mods you're using for new versions after hotfix 16, and disable any others for the time being.
