Report a translation problem
I did have occasional ambient screaming, but it always went away when quick travelling. Not sure if it helps, but might worths a try.
The Astarion bug though. Please be a good member of the community and make us a screenshot!

Thank you this is a good idea! I'm sure they know about it already but no harm in throwing my hat into the ring!
From all the bugs I encountered: the combat inspiration of the valor bard not clearing off normally, a certain Act III drow leader dialogue not triggering, crashes to desktop in a certain house in Act III and finally, unability to continue the game after clicking on a dead character in Act III... that is really the smallest issue.
It was weird because I think there are more bugs now than at release. That being said, they have buffed some of the bosses, which is a welcome challenge.
