No one left behind Achievement. (possible spoilers>
Now a friend has just gotten this one and I thought that I had gotten it as well on my last play through but it seems I didn't. So I checked the Wiki and it mentions something about an attack on the inn; which I'm taking to mean the last light inn; and this attack never happened during either my current play through or the last one. Is this the attack that happens when get go to Isobel for the blessing? And if this don't happen does it mean you can't get the Achievement?

The attack is irrelevant for the achievement (assuming no one dies during it).
Keep in mind that Roland will die if you don't follow and find him in the Shadow Cursed Lands and that you need to save every tiefling in Act 1.

Also Zevlor can't die during the final fight.

I can't say for certain but it might be a combination of both the Grove fight and the Last Light Inn fight as the description I see is

Save every Tiefling refugee in one playthrough.

Yes, there should be an attack when first talk to Isobel - from the Flaming Fist guy.

IIRC in my last play through zevlor had died that must have been it.

True the only thing is that I've never had a reason to take to Isobel due to having killed the dryder and his party and getting the lamp from him. When I got to the last light in I already had the Pixie's blessing so didn't need Isobels. And because of that there was on attack on the inn.

Another one is the Tieflings with the Goblin prisoner in the grove. If you let Arka kill Sazza she will die later too (I always miss this one )
