Report a translation problem
Okay, so I found out you can go back / repeat some Shad interpersonal dialogue you may have taken different options with in the past. Just use the disguise spell/item thing and talk to her -- the 'get to know you better' dialogue tree opens back up (it was gone b/c I had already done it). One of those options allowed me to hear (in this playthrough, at least) the bit about her memory wipe.
So -- great -- I thought I was in business. But she still won't eat the noblestalk, I can't talk to her about it or anything. All she does is refuse to eat it without talking about it first. Odd.
I then went back to the moment of the turn in with Baelen (reload). Derryth fesses up that she's looking for noblestalk, it is in my inventory, and I have no option to give it to her!
Something's off here. I've always been able to give the noblestalk to Derryth. What's going on?

For whatever reason if I get the noblestalk...
1) ...before saving Baelen
2) ...after saving Baelen but before 'turning him in' with Derryth
3) ...after turning Baelen in to Derryth and chatting her up about the noblestalk
...I can never turn the noblestalk in, while it is sitting in my inventory.
Reeeeeediculous. I've turned it in to her in at least 5 other playthroughs.
This isn't a Shadowheart noblestalk problem, it's a basic 'game doesn't think I have it' bug.

For mods related issues consult the mods authors.
This seems like a more clear cut bug than anything else.

-- Find the mushroom and have it in your party's inventory (duh)
-- Learn about Shadowheart's "sealed memories" - there are several events that lead to that, the easiest way is probably just to ask her "tell me more about you" when your approval is high enough. If she tells you "I don't know much, my memories were sealed off, yada yada" - you know you're on the right track.
-- Learn about the effects of the noblestalk - the safest way is to:
0. Save Belen and see that he reaches their Myconid's place (duh)
1. Ask Belen why he needed to mushroom (he will tell that Derryth needs it for a brew)
2. Then ask Derryth what makes the noblestalk so great, also ask if it could cure Belen's condition
3. Not telling either of them that you've found the mushroom (this might be now irrelevant after some patches)
After that you have all the information you need, so you need to talk to Shadowheart and ask her to eat the mushroom. You cannot just "use" it via inventory even after you've learned all the information. You will still need to pass a speech check to do it. If you did everything right, she will tell you she remembered some stuff about the cloister and the tiefling Rennald.
^ works even with 90-something mods installed.

I slept a few long rests consecutively to rule out the 'I'm just ahead of her plot readiness' and that's not it. She has told me everything, I know about the memory wipe, I've seen the shared vision with the wolf, etc. I still can't give it to her.
I honestly think it's a bug. I've done everything you listed above, Derryth is always asking me for the noblestalk after returning with Baelen, it's in my inventory and I simply have no way to give it to her -- no dialogue option, can't sell it to her (which would be wrong for a quest anyway), etc.
So it's not a Shad timing issue. The game is not triggering a turn in dialogue or doesn't believe I actually have the noblestalk.

Honestly wondering if I should just go back to right before doing the Baelen quest and continuing my playthrough until this gets sorted. I think Baelen / noblestalk is on the table until Moonrise, so I should be fine to finish the under and do all of the Mountain Pass before I have to loop back, right? Maybe by then they have a solution for us.
And even if this aspect of the playthrough isn't working, it won't bork Shad's personal quest anyway, right? It's just added flavor I'd never seen before. I could even youtube if I wanted to see it badly enough.

The game just does not recognize that you have the item, preventing you from turning it in via dialogue.
The ring is in my hands and the quest log still says 'Get the ring from omeluum' -- and every time I talk to him he asks me what I will offer for it.
Ugh. This is more than Shad, this is more than the Baelen quest. I may be hosed until this gets sorted.

I only have 5-6 and they are a weird mish-mosh -- an plot related injury healing thing for Lae'Zel, a wider hotbar, a store, etc.
But the simplest possible answer worked. Shocked the reddit thread on this blew up for something with such a simple fix.

No, but I see a lot of people on reddit pointing to that.
I'm guessing a number of mods that tinker with party makeup or inventory would naturally run into trouble with the new party sharing inventory thing this hotfix released.
