Us missing.
So I saved Us at the start of the game and Us was alive as the ship crashed, I'm now in the Nautiloid and the cage where Us should be it isn't.

Did I do something wrong by doing [DEXTERITY] Mutilate the brain or do you need to miss this step?

Any help would be welcome.

Assuming I understand your challenge:
Every time I successfully encountered Us was when I didn't mutilate him. As far as I am aware when you harm Us that way it prevents Us from surviving to be in the cage.

I mutilated Us every time and has always been in the cage for me...

Same, if you "neuter" its aggression, it should show up in the cage.
