You spoke to mizora after the coronation to get information about the duke ? With no infos the game probably assumed the duke is dead, you can still probably found him in the iron throne or have extra clues with mizora in camp.
As long as you don't kill Gortash before you find out about the iron Throne (through "Avenge the drowned" or just wandering aorund/exploring) you should be fine.
Mizora is lying, she knows Ravengard is still alive in the Iron Throne, (*) but wants Wyll to feel guilt over breaking the Pact so he will come running back to her. Or maybe he's still alive, but come back to contract my pet and we'll tell you what's up.
(*) in most game states.
The new thing post-patch 5 is I also run into FF accusing me of "killing" Ravengard, who I tell is of course still alive (granted that's meta knowledge), and they tell me they were put up to this by Mizora. Well, good to see you're listening to tips from devils, that's fine police work, there, mates.
Mizora complicates the rescue, but it's still entirely doable and having done so will be recognized (e.g. directing you to the next stage of Wyll's companion quest, and follow-up dialogue with Mizora).
If you try and rescue Ravengard after Wyll broke Mizora's pact, she will appear when he comes out of his cell, and sic some exploding spiders on him. They are little threat to you, but be cautious, throw healing potions on him, and kill them when he's not nearby, as these exploding spiders will definitely kill him.
After all this drama, she just hangs out in your camp near Wyll and Ravengard, possibly offers to have sex with you, and is just annoying.
Then Mizora outside the fort told us that "he's not quite dead" which left me confused. After that, she came by in camp and offered to bring him back from the dead in exchange for Wyll's eternal service which we refused. In the end, the dad stayed dead, Wyll became the sword of Avernus, and Mizora helped out at the final battle.
I was never able to find the Iron Throne.
