Does it mean anything when characters say they need sleep?

It means you should rest soon your spell slots are almost gone.

You can ignore them.
A lot of companion quest stuff gets advanced over long-rests. So the game just tries to give you hints to take long rests every now and then.

I believe that at one point the game may have had an exhaustion mechanism, but if so it got dropped. You can just completely ignore when your NPCs start whining about wanting to rest.

The main reason to rest is just to restore your HP and spell slots.

When you say you're tired from playing too much BDG3 and want to sleep. What do you do ? Do you play an extra hour or do you go to sleep ?

It just means they have spent some of their class resources. No additional meaning.
