Report a translation problem
1.) When you play the game, make sure you open the launcher. Choose to play BG3 on Vulkan. When that is loaded, set up your resolution/graphics as you want and hit 'Back' like usual.
(I noticed on another board that a user was able to keep their settings on Vulkan instead of running it on NVidia)
2.) Next, go ahead and quit the game. Reload it with the launcher and load it on Vulkan again to make sure the settings stick.
3.) Quit the game. In Steam, right click the game on your list and go to Properties. Under Mod Commands, put in ' --skip-launcher ' this will skip the launcher when you hit play again. Close the properties setting and make sure --skip-launcher is still filled in, if you need to.
4.) Launch the game (Which should NOT load the launcher and jump right into the game). At the same time, it should default to Vulkan without loading the launcher, and thus boom. Should be "fixed"!
This may be a Bug due to a typo, or simply a field that was not filled in for the devs (Which isn't really a bug and may have not been scoped out during development, or simply an issue that needs to be handled by NVidia instead).
When BG3 loads, it must fetch the graphics info from NVidia/GeForce. I noticed that in the 'GeForce Experience' it has settings for everything from the resolution, graphic quality, but it does NOT have one for Aspect Ratio.
This leads me to suspect that when the game loads, it retrieves all the details, but it's missing the Aspect Ratio setting, so it defaults to some other setting. (I noticed behavior was fixed in NVidia mode by changing the resolution back and forth and it just fixed itself)
The reason why Vulkan works is most likely because that driver DOES have the Aspect Ratio setting available to fill out and retrieve, which explains why that works.

Conclusion: I'm just guessing that either the Aspect Ratio field was not added in by the devs or it's typoed (And the naming convention of the field is different between NVidia and Vulkan) OR simply, NVidia doesn't have a Aspect Ratio field on their end to save.
