It depends.
My Karlach Throw setup is Kushigo Gloves + Flinging Ring + Returning Pike or Nyrulna, then give her Caustic Band or Callous Glow to add extra damage on top. If Menthol is in your party, she can Brand Karlach for extra throw damage (supposedly it doesn't wear off after the first throw but still adds its damage bonus to the throw), and if not you can have someone nearby - preferably a Lore Bard with Crusader's Mantle on the BoB build - to give her damage amp.
If, however, you need her to tank . . . then she'll want Sparkswall, Darkfire Shortbow, some kind of medium armor (Karlach can still rage while wearing medium armor and Adamantine Scalemail is lowkey kinda cracked), Helldusk Helm, and then a Ring of Flinging with whatever gloves gives her added utility.
Honestly tho, Bracers of Defense are kinda bait. Everyone either already has shield proficiency (which means you don't lose the glove slot for defense, plus there's like 3 shields for casters which is kinda pogs), has armor proficiency (which negates the bracers entirely), or is a Monk (who wants either Cindersnap or Sparkle Hands before getting the Soulcatching Gloves).
