The Emperor tries to get intimate with you not because he's lonely, really cares for you, or distraught over the death of Duke Stelmane. It's just another attempt to manipulate you. Of course, my viewpoint is he's extremely manipulative in general and he figures this is the next step in doing so.
He killed his last BFF Ansur, Wyll strongly hints that Stelmane suffered a stroke it's possible the Emperor triggered, and BTW though I've of course never done it, apparently if you accept "intimacy" with the Emperor, he never talks about it or discusses it ever again with you.
Rarely ever feel bad about freeing Orpheus, and I tell him he is the manipulative scum he really is, whether he really WAS heroic Balduran at some point, ever.
I also suspect what's really happening is actually taking place in your mind and not physically happening. Kind of like how "David" slept with "Syd" in "Legion" (the FX TV show). He even will readopt his Dream Guardian form if you ask.
