I know, but as I said: I don't think it has EVER worked.
It doesn't use up any resources beside the cleric's Reaction so IMO there's really no reason not to use it. My cleric can't hit ♥♥♥♥ with a melee attack, anyway. ^^
When Shadowheart gets attacked, I get a message asking whether I want to activate it. So, no.

It works exactly as intended. The enemy rolls twice on their attacks and takes the lower roll.
If you have low AC then they can reliably hit you, especially on higher difficulties when they have higher bonuses. It also won't apply if they are doing abilities or spells that require you to make a save because they aren't attacking you.
It was as if it didn't roll that second dice for disadvantage. The community came up with a temporary fix though:
Go to your OPTIONS MENU under GAMEPLAY and DISABLE KARMIC DICE. It's near the top of the menu.
The problem with this fix is that you now sometimes have to use like three inspirations to pass a skill check in dialogue. I will test later today if I have time with the holidays and all that.

I would advice you to disable Karmic Dice for now if you're running into problems.
