Over a period of 3+ years of public EA, the systems they added in small groups became integrated at deep and superficial levels. (I was there. It was very interesting to watch.) The result is that a tiny bump to data manipulation can have game-wide effects.
I haven't tried, but I also keep reading about modded games preventing achievements. I wouldn't know. I promised myself I would mod after one playthrough, but the one time I did many, many playthroughs later, I went back and started over after a couple of hours without any mods. So many other games I've modded to heck and back, but BG3 just makes me want vanilla. (Trivia: I don't like vanilla flavor IRL. Yuck.)
the first time in the dialogue of the unholy assassin of bhaal i chose that i was ready to stand trial, but then we have to describe how we killed, here i refused to give details and i was attacked. after that i killed Sarevok, and there was no achievement in the battle with Orin
i replayed this part and attacked Sarevok right at the beginning of the dialogue and after that i got the achievement in the battle with Orin
i don’t know how it works, but this is the only thing that helped me - dont step onto the way of an unholy assassin at all, even if you plan to refuse later and just wanted to read the lines
