Report a translation problem
The reason I think this happened is before patch 5, there was horrendous lag, meaning I had to wait ages for the cutscene to load, when it did, it teleported Astarion out of range, so the game thought he was not there. Maybe something like that happened with you?
I would reload it if I were you, though you can convince Astarion you were just looking out for him, but idk if you need high approval/romance for that (just seen it on YT).
If one of the spawn dies in the fight, he cannot ascend.
Triggering the fight without the cutscene shouldn't prevent the quest completing, but it's more interesting if you do it properly. Having Astarion on the stairs at that point will prevent Cazador from capturing him, but will make the first scene behave like he's not there.
If he is captured, you can free him with the help feature. If it's too hard to move a character there fast, switch to someone else during the cutscene and move them there already.
