Report a translation problem
I had just recently saved two games shortly after doing this too. Then this patch dropped, and after loading one of those games, my very next camp scene showed Wyll, as a devil, confronting Karlach.
As if it wasn't bad enough, that letting Karlach go through ceremorphosis is seemingly the only way to let her live a happy life, while also sparing the life of the githyanki's rightful heir and increasing the odds of them overthrowing their corrupt evil tyrant ruler. I had really hoped I could do those things, while reuniting Ravengard with his still-human son.
What's the point in having your cake, if you can't eat it too?! If you're not going to let me eat it, then don't give it to me!
The problem you list is just scripting... games with scripted branches go down one of them regardless, or they crash, and this case the logic sees that she is alive & in party. Likely killing her didn't trigger the dialogue and so that state didn't register, so it does the best it can with the state the game is in. Could be better, list it as a bug...
