Reinstall does not clear out folders for mods or certain settings files, either. You'll need to do extra cleanup to get rid of mods.
Unfortunately, I have yet to use mods after release, but this was pervasive in EA, requiring to delete folders and edit files related to mods. It is my understanding that folders and files are not all in the same place as EA.
Mod managers can usually handle that stuff (but I did mine manually back then—I like to know what the apps are doing for me rather than just let the app do things I don't know).
Just be careful to not delete your profiles with your savegames.
None of my guides tell anyone to delete their savegames.
Did you figure out your issue? What fixed it for myself and my partner was removing the "Improved UI" and "Improved UI Assets" mods from our mod manager. Idk what you use for your mods, but we use BG3MM and it was an easy fix.
That said, both of those mods have been updated on Nexus for patch 5 and I reinstalled them with no character portrait issues. Try uninstalling and updating those mods, if you have them.
