How to disable dice roll

Are there any mods for that so it doesnt show up on my screen , it breaks the immersion when it does show

No, there are none - for a simple reason that if you disable the rolls in the meaning of hiding the UI for it, then you'll have no way of knowing if it was a success or failure. While in many cases it will be self-evident from the way the action/dialogue choice unravels, it is not always the case and it could easily confuse or break the immersion even more.

If, however, you mean to disable rolls in a sense that you want them to just not be part of the game (auto-success, I guess?) then that'd require some engine change altogether. Not that it's impossible but it's like trying to mod the game out of the game.
