Besides, NG+ in itself really doesn't make any sense in a CRPG, in my opnion. Yet, it worked for Wticher 3 but I don't remember if there is a max level to begin with, despite having done a NG+ run myself.
I find your comment pretty ironic, due to Baldurs Gate (1 & 2) having an NG+ since the get go.
Same with Icewind Dale, and Neverwinter Nights. Planescape: Torment is the outlier when it comes to NG+ here.
As for "how would it work", probably the same way as those games; Importing\exporting your character as it currently is (level and gear).
Game becoming more difficult with 'NG+', is a more a soulslike thing, than an RPG thing. Most RPGs with NG+ has always been a power-fantasy 'reward' for when you beat the game (Infinity and Aurora engine games being an exception, as import\export is available from the get go).
How GM mode will work is anyone's guess... if it'll happen, but some think it's on the way. A character import/export feature would work well with a GM mode.
