1. The known workaround for full party custom characters works in multiplayer as well - you just need to navigate through the steps smartly and see whether you manage it with your communication settings in one go or by sharing your savegame.
2. Yes. Full custom party possible with just two players, as stated above.
3. Yes. It is bad enough to consider to take at least one of the NPC companions with you to get most out of optional stories.
I would either go straightforward with starting multiplayer with two game instances each for character creation, or go with sharing savegame until all characters are created after you started it as two.
So as I understand, the only way for 4 custom characters is to do a workaround in the beginning to make the game think we are 4 players, got ya.
Otherwise we have to take 2 original characters but we could completely respec them into other classes that we want? It's just that we cant change their appearance?
Correct.
I do not recommend taking origin characters for your first playthrough.
You could also start with two custom characters and then see first if the NPC companions are to your liking. Additional custom characters can be added later on too, as long as you do not start as an origin character. Origin character cannot be dismissed so it might be problematic if you do not like using them anymore later on.
we both really want to make 2 custom characters each, we don't care about the premade origin characters or any dialogue we may miss, unless anyone has any good reason that that stuff is unmissable.
now we just have to decide party composition!!!
The good thing is, you can always put away at least two of the custom characters and take them out at your convenience. So no pressure on that front.
Similar with the classes - you can respec later on anytime. Not only origin characters but everyone. Starting out as a Fighter but not sure if you are happy with it? Respec to Paladin later on is no problem, and so on.

I dont get it, so how do you achieve to play with 2 custom charaters each? Can you describe step by step please?

works also in multiplayer by having your friend connected via the ingame code and then you as a host create additional instances. That is how you add origin characters
Otherwise you could also first start the campaign as two, then each of you add a character by sharing the savegame.
