You think you are at the right place to ask for something long and hard?
It depends.
If you're doing all the quests + playing on Tactician then this game will easily go above&beyond 120 hours
Difficulty is questionable, really depends on how you play and how much experience you have: I find the game easy, but other people find the game quite hard.
The difficulty depends on how much certain elements, such as dual-wielding hand crossbows, are used. It might be challenging if you are not familiar with D&D 5e and Larian combat design (it is probably an ambush).
Difficulty is subjective, how familiar to Crpgs and D&D are you? If this is your first, will probably be difficult even on Balanced. Veterans and min-maxers around here tend to complain that Tactician's way too easy.
Tactician , all quests , read every book , mid act2 , 50hrs. So I think 120 is a little bit of a stretch. Maybe if you just run around enjoying architecture or whatever.
If you are thinking of playing MP with your friend, play for the fun. To play as a serious game, create your own campaign to play solo.
If you know how to make a build you are in God Mode , else you may struggle
You should consider EVE Online which surely fulfill your demands in being long and hard...
co-op is hard to estimate, it depends on your interaction with buddy, but it would certainly be longer
you get 3 levels of difficulty to pick from, the options are good despite some vocal minorities complaining, especially on first playthrough (you can also change it in game any time)
