They have to draw the line somewhere when it comes to ESG and budget.
When you are rigging a mesh, you need a certain level of "predictability" in its movements, body ratios, number of bones (which do not relate to biological bones), that allow for a animation to be "rigged" into a mesh.
The more they allow you to make different types of characters, the more work must go into rigging and animating it.
Of course you have games all around doing it, but have you notice how many of them you can do what you describing ? You can change a female body to look like a femboy, but you cannot make a male body that looks like a female one, and that is just a matter of having certain predictabilities on how animations and rigging works.
For that to work "better than the already garbage that is the whole rigging of characters in this game", you would need specific animations and specific interactions not only with the character but from others to them, which would be the same work already done to make the different female and male animations. You will not have a good result simply making a female animation on a male character, no matter how "feminine" it might be made look.
And then that would be a "waste of time" of sorts, because it is a very "niche community" to hold the entire game for work on.
Not even cyberpunk does that, and if there was a game that could be "in the niche" for that would be it.
And why is there no option to have proper breasts, but only tiny, 12-year-old microtits?
Damnit Larian... it is a terrible terrible game if I can't play a futanari with Knight-Cups and a gigantic horse-schlong.

Just equip armor on current male and female companions. It's not a heavy lift to make it work. Video games have been doing it for 20+ years.
OP has a legit question about character customization. Why not have this?
