Vlaakith and her weird decisions *Contains Spoilers*

If you deny Vlaakith order to kill the Spirit Guardian inside the Artifact (or Orpheus since it's what she actually mean), Vlaakith uses Wish Spell and ending our game (Wiping your party). But if we obey her and enter the Artifact realm, then decide not to kill the guardian, she doesn't use her Wish spell but rely on her troops to kill us. Now, there's an argument that she can only cast the Wish spell if she speak to us or sees us. But then, when Laezel talk to Vlaakith one last time in later act and disobey her order once more, she didn't user her Wish spell even though she's clearly pissed off. Did she changed her mind or this is a bit of a plot hole ? Is there any explanation that made sense ?

She only uses wish if you question/mock her divinity. you can deny her command just fine, in which case she sends her guys after you immediately. (or impotently flails if you already killed her guys)
in short: she only really wastes her wish if you put her in a situation where she's either feels like she gotta prove her divinity (by putting on that show) or you piss her off enough.

Edit: Forgot that immediately blaming SH for stealing the artifcat makes her wish you dead too for some reason. That one I cannot explain. There might be other edge cases.
