Proper use of Reckless Attack? (Barbarian)
I'm not 100% sure how this works, are you supposed to use it as an attack to gain advantage or wait for the reaction, is there an optimal way?

That's up to you really; it would be a waste if you already have an advantage on an enemy however (like if they're already prone for example). Use it when you REALLY need to hit and pray it does.

When you attack, you may miss. You will be prompted to use Reckless Attack. This allows you to roll the attack again and take the highest number to see if you hit.

Doing so will allow every character that attacks you to do the same, however.

So my rule is to use it whenever prompted unless my barbarian is for sure about to take a bunch of attacks from multiple people. But, usually even then I still do it whenever prompted.

I leave it as an automatic reaction. It only activates if the attack is going to miss.

Nope. You do not have to actually hit anything to maintain Rage, you just have to attack something or take damage.
