The main problem with doing that is you also lose Dammon and can't do the second part of Karlach's quest (unless he somehow manages to survive that but I don't THINK he does).
First gives a ring to summon minthara at party.
Second, my preference, allows you to knock minthara out at goblin camp, and rescue her at moonrise as you do in the evil route.
Works well in say, a redemptive durge run, her and karlach are quite a pair.

Don’t get me wrong, mods are amazing, but it’s not worth risking losing your entire campaign come an sudden patch.
I heard they introduced a patch that kills Halsin if you have her, I think. Idk, I remember hearing about it but I havent done that
I actually dont know where he went in my evil run so I'm a bit worried
Losing the rewards you get from saving the tieflings at the end of their entire quest line, as well as losing all the stuff you can buy from Dammon--all of that is worth far, far more than Minthara to me.
This is after doing a full-evil run with her. Don't get me wrong, I like her, but I like owning every piece of mid-to-high-end gear, more. Much more. It doesn't help that her story, content, and any semblance of character progression essentially end right after you save her from Moonrise. Prepare to go, like, 30 hours with virtually no meaningful interaction with her, after Moonrise. Only at the very end of the game do you remember that you actually romanced her, because her relationship interaction is evidently broken.
Allowing the tieflings to die will ensure, at the very least, you won't pick this up:

And wouldn't you know, I was a warlock on this run. I really wanted this robe, and I expected to get it in my evil run, but I didn't remember exactly how I got it on my earlier runs, and I didn't look it up. Cut to me being disappointed when I was midway through act 3 and hadn't found it yet. Then I looked it up.
It's a game, nobody dies, so I decided to go for the companion that I wanted to have. Frankly, I find it annoying that her romance is gated behind that arc, but whatever.
