The reason he himself isn't hostile to start with is because he's an evil overlord and therefor genetically incapable of engaging combat without first giving a monologue detailing his plan.
What do you think was gonna happen?
I assumed there would be at least some dialogue with the guards there. I mean why aren't the guards in the city attacking me then for the same reason?
Heck even the civilian/nobles attack you in the Fortress.
This seems like a very unfinished piece of content where yes Gortash did go hostile as he promised he would, but the layout and interaction in his location weren't changed. They just made everyone on that map hostile and put a ribbon on it.
... oof.
Sounds incredibly tedious.
Not to mention sadly it doesn't seem to reflect story or game-play wise.
They are pretty much considered as dead as far as the game is considered with no unique interaction, story bit or even gameplay benefit to you. Unless you do it for your own self RP, it's a long waste of time sadly.
How I fixed it:
Instead of going to the prison, or head on assaulting the whole castle, I went south of the draw up bridge, there I flew over to the cliff. there's a ladder there and some vines which takes you up to Gortash Hall.
From there everything worked as it should. with dialogues and Kalach's quest
However -
Spoiler:
you can go outside and circle around to a back entrance and come up that way .. and from there you can go out on a turret and jump up to scaffolding and then ladders to get to the level Gortash is on skipping all the mobs in the Coronation Room. You could also skip the fighting on the first level by Misty Step down to the outside path.
That’s true, but is just a workaround.
They remain hostile still afterwards so you can’t walk around there at all.
And it doesn’t actually change anything if you spare them or not ( in fact there’s practically zero consequences other than the last 2 choices for the endings sadly).
