2. aura of courage prevents frightened, not fear. if it's not working against frightened that's a bug.
1. I think it used to be like you said, don't know on 5E.
On https://www.5esrd.com/spellcasting/3pp-spells/spells-open-design-llc/shadow-step/ it says it "V, S" though. So I'm guessing it was based on it.
2. Not sure if this is relevant:
Aura of Terror
"If you cast this spell when you already have the frightful presence ability or a fear aura, the existing ability becomes more effective in the following ways:"
"Creatures that would normally be shaken by your fear aura are frightened instead, and creatures that would normally be frightened are panicked."
The Aura of Terror is specifically from Act 3. I know Larian changed a few things from 5e to bg3.
But in BG3 it specifically reads:
-Nearby creatures have disadvantage on attack rolls and saving throws.
-Creatures that are immune to FRIGHTEN negate this effect.
Yet a paladin with Aura of Courage that makes them and nearby allies immune to frighten, still suffer the effects of Aura of Terror, which makes a specific part of Act 3 much more difficult for melee classes.
