How to get the hair?
I am at wits end... every source I can find just says to get her to 10 health or lower and she will offer you a deal, but she absolutely does not do that for me. I've reloaded this fight a dozen times and even left her on 3 hp for 5 rounds in the hopes that she'd offer me this deal...

How the heck do you trigger that conversation? There has to be more to it than HP.

Make sure to save Mayrina from the cage and use the button to open it. It wasn't proccing for me either but after i did that it worked fine

Are you letting the round cycle back to her turn? Try and last for a few rounds when she's low then and see if it happens.

I let 5 rounds pass and nothing at all happens except she attacks me until she runs out of spells and then starts slashing me.

I tried that too, but in every fight she teleports Mayrina out of the cage as soon as I kill her illusions.

Weird, I haven't done that encounter since the latest hotfix. Did you do what PaloG suggested? Normally, Mayrina is out of the cage just naturally in the encounter. She didn't die or anything right?

The only thing that is... I guess special about my game is that I started it with a friend, and due to scheduling I had to put their character in Wither's wardrobe. Is there like a "flag" that says this can only trigger in single player and it just can't happen in a multiplayer game that has been turned into a single player game?

I have no idea. I wouldn't think so, but I've never played multi-player in this game.

You could try reloading and doing the encounter again, but I'd submit a bug report to Larian and make sure to upload your game save in the process.

Thanks, I think I will try to let Larian know.. I have tried doing the fight several times and I never get the dialogue to trigger.

Yeah, it sounds like there's definitely something going on there. I wish I had a campaign in Act 1 to test it myself, if it was a multiplayer thing or not.

Maybe someone else will see the thread and chime in. Could always skip it for now and do some other things in Act 1 if possible. Or just kill her without getting the +1 Ability Score.

It wouldn't trigger for me if she was silenced. I had to let her go through with all her shenanigans to get it. I think you have to let her impersonate Mayrina.

It definitely can happen in multiplayer. In my coop run she gave us the hair, but in my solo run she didn't. Granted, I didn't "try" to get it. Maybe I just killed her too fast

Are you killing Mayrina in the battle? She disguises herself as Mayrina and if you hit the wrong one she dies. You don't get the offer if the real Mayrina was killed. Either switch to non-lethal attacks to knock Mayrina out or give Ethel a status effect before she disguises herself so you can track her. When I did this quest I had to reload because I killed Mayrina by accident while Ethel was disguised as her.

Call the hag's bluff and hit the cage to see what happens. In our game she pulled her out of the cage and immediately started a conversation with us. Everyone tries to save the girl that doesn't want to be saved...she's not begging the hag to stop and go away.
