I'm wondering what is the best thing to do, I'm in ACT 1 still, level 4 party, but I read she is also in act 3, I'm a but confused about what to do next.

If you kill her she will revive and be mad in aact 3, dont know what happen if you are coooerative in act 1 though

spare her it prevents possible bugs from happening in act 3 you can rescue the girl though if you pass a check and still get the stat boost.

Best thing to do is fight her and do not kill her, get her down to 10hp and when its her turn she surrenders and she will give you 1 point into any ability of your choosing its permanent.

She's only in Act 3 if you resolve her quest in certain ways. So play how you want and roll with it.

Metagaming info, heavy spoilers for optimal gain from this questline: It is beneficial to do her fights and free Mayrina in Act 1 in a way that the hag will also give you the +1 ability score permabuff and not let her keep the baby so you encounter her in Act 3 as well to get her drops there. If she keeps Mayrina/baby she never shows up in Act 3

I got her down to 5 hp and she did not surrender and just left. Also there is the issue of four of her people coming after me.
