Report a translation problem
Dissapprovement should be correctly towards Astarion. Even if he is an NPC he can be controlled like an origin character or any other companion character. You can easily check it by those who gave him disapproval - his avatar should be seen there in the approval bar.
the game take it like you forced astarion to lose his eye for you. for what i know karlach dissaproves it, you can solve this if you dont have everyone in the camp, by taking them all out of the camp and keeping astarion alone there, gale and wyll dont care about the eye. but karlach does, idk if another of them do, so take them out of the camp.

Yes I was planning to take Gale and Karlach out of the camp for this.
But the game really shouldnt take it as I forced it, it is Astarion who does it, its not like Im convincing him to go with the surgery.
The dialogue options are not out of my mouth but Astarion's.
When it says "Clamp your fists and bare with the surgery" it's not my main but Astarion.
So as far as companions concerned it should be Astarion.
