Derivation cloak does not work?
So i have astarion equiped with cloak. i start combat by coating weapon with basic poison. But every time opponents fail save and become poisoned - there is no healing happenning. Am i missing something?

What does the derivation cloak do? Improve your ability with maths?

I just noticed the same thing as OP. The cloak does not work.

For the record and people that don't know, the cloak is supposed to heal you 1d8 every time you inflict poison damage. Or at least, that is what the tooltip says, which could be lying and wouldn't be the first.

Have you tried using it with a spell? Ray or Sickness or Chromatic Orb? It might not be recognizing actual poisons for some reason.

If what you speaking is true, than there is something really wrong, cloak says

When you Poison a foe, heal yourself for 1d4 hit points.

I didnt tried it with poison spells but i will.

Which does not close the issue, cloak is bugged, either its functionality or its description.

I think it's supposed to trigger when an enemy receives the Poison condition, not just poison damage. And OP said he was specifically giving them the Poison condition via their failed saved throw.

So yeah it sounds broken currently.

I had the same as OP, I coated my blade with a basic poison that rather than damage, it gives the condition "poisoned". Still nothing. I'll try with spells or similar.

Probably only works when you cause the poison via spell or smth and not weapon coating.

Its kinda poor item in this case since poison cantrips are not well rounded (poison spray is meh in most scenarios) and building around poison spells for 1d4 healing sounds stupid.

Derivation Cloak is a rare cloak that heals the wearer whenever they deal poison damage. Cloaks can be equipped to characters back in their designated inventory slot and provide different effects ranging from increased Saving Throws, to gaining Advantage on certain attacks.

It shouldn't trigger on poisoned state but with poison damage.
It's a bit of a beast to code this I think, so I can see it getting bugged.
Best report the bug to Larian Studio through the launcher so they know to fix it.

Not really. Since there are different types of damages, it would probably take about max 20 rows of code. Either if it works with damage or if it works with status application.
All you need to do is perform basicly the check if item is equipped, if user has made certain kind of action(with damage type it is even easier since you wont need the status affiliation check)...so sorry, but no this should not be that hard to code.
Unless they managed to get everything else so unnecessarily complexly coded that they are unable to read their own codes.

Update on this topic. Have the game up with one of the cloaks in my inventory. "Poison" in-game is hyperlinked to the condition, rather than the damage type. So the cloak only triggers it's healing in response to the "poisoned" condition being applied to an enemy, from sources like noxious cloud. Which also means they have to fail a save against the condition. None of the common "poison" items work, since they just apply extra poison damage to the weapon.

This makes it a largely worthless item, at all good only because cloaks are rare af in this game.

Poison dmg does not equal poison i take it, there is also a poison that simply gives the effect of being poisoned.. Depending on the wording on the cloak, when you poison or when you deal poison dmg one of the 2 would not trigger the effect.
