I haven't reached that point in my Durge playthrough yet so dunno if it differs from general custom Tav but have you been to the Tribunal yet? That's what normally gives access to the Temple of Bhaal and does not require all the hands.
It is somewhere in the Lower City, yes, and not behind that gate. Since you're doing the murders yourself I suppose you've confronted and killed Dolor, the red clad Dwarf(unless again something changes for Durge) in which case there should have been a letter on him to point you in the direction.
If you just wanna progress and don't care about searching for it yourself I'm gonna give instructions in a spoiler. If you wanna look for it yourself do not read below. The entrance to the Tribunal is in a tombstone making shop next to the Graveyard. You have to go to the back room, remove the portrait from the wall and click the button to reveal the entrance
