Specifically, I found this thread while hunting for info as I try to determine whether *I* have encountered a bug on the Karlach romance, or whether I have (like you) likely failed to satisfy the right conditions for a third time. This time, when I got to the tiefling party, it seemed clear that I had finally managed to do things correctly: when I approached Karlach first at the party, I was able to proposition her and see her adorable reply. We made plans to get together after everyone else was asleep, and I was feeling very pleased with myself. When I went to sleep and selected Karlach to spend the night with, however, the game skipped right over the romance scene and launched me directly into a scene with Halsin about the shadow curse the next morning. Karlach seems to think we spent a great night together, so unless I've somehow inadvertantly set my game to PG, this feels like I may have encountered a legitimate (and legitimately deflating) bug... a coding error. I'm hoping there's still some way I can figure out to circumvent it. Keep fingers crossed for me, if you would.
As far as your problem goes, though, I'm not sure if you're eager to try to follow my course since I am in a different kind of celibate hell with Karlach... but it does seem like I successfully met all her romance conditions this time, like the game intends to keep our romance marked as active through later acts. And I reckon my two playthroughs' worth of experience trying and failing to properly meet those conditions is decent enough fodder for meaningful comparison. So if you're interested, if my failures can be useful to you in some small way, here's what I did this time that seems to have kept me on the Karlach romance route:
1. I had "very high" approval with her at the tiefling party.
2. I had no more than one active romance with another character going during Act 1. You might be okay as long as you break one off before the party, but to be safe I'd make sure to limit yourself to one (or better yet, zero) active romances before reaching the party. As far as I can tell, the game will not let you engage in more than two simultaneous active romances at any given time. During Act 1 it won't force you to choose between them, but once you have two going the characters seem to lose any romantic interest in you they might have otherwise had (unless you break one off in time to not miss a condition for one of the others). And since Karlach's romance MUST be started in an Act 1 scene, and relies on a string of very specific dialogue options at certain points, you should probably play it safe to ensure those dialogue options show up when you need them to if you're serious about going for Karlach.
3. I spoke to Karlach before speaking to anybody else at the party this time. In the past, I didn't do this; and she never came on to me at the party. In my last playthrough, I did get the option to try to come on to her at the party, but she basically just laughed at me and said "yeah, okay, whatever". Ouch. It might also be fine as long as you reject any other advances at the party before talking to her, though.
4. Make sure you've kept Dammon alive and safe, you've spoken to him at the Grove with Karlach, and he's given her the first heart upgrade (requires infernal iron). In the dialogue that follows, mention that you want to "finally be able to touch her", or something like that. There's a dialogue option that refers to touching her, and I chose it.
5. Not sure how much this matters, but I had her with me when I confronted the "Paladins of Tyr" and used the "create water" spell to douse her afterwards once she started burning up (and burning the place down). I noted that the game showed me that this created steam all around her. In my other playthroughs, I let her just go nuts and burn everything down.
6. In the conversation right after the poser Paladins/ create water scene, she had some dialogue about how she hadn't been able to touch anybody for ten years. I said "you must be really pent up" or something like that.
7. I didn't have her in my party during the goblin camp massacre, because on my last playthrough she got really upset at me when I was going around pretending to be a True Soul in order to figure out how best to go about killing everyone. Apparently, she thought I was doing an evil playthrough for a minute. Took a big hit to approval for this; and even though it was still "very high" at the tiefling party, I think she might have ruled me out as a romantic option because of it? Not sure if it made a real difference, but I am sure that leaving her at home worked out.
In any case, I hope this makes it easier for you to isolate whatever it is you may have missed in order to make it into the Hellion's Heart. If anybody who stumbles on this has any info regarding my skipped post-party romantic sequence, I'd be thrilled to hear it...
Thank you very much for such detailed comment, I'd like to say that I hope you finally succeeded in this hardest BG3 challenge aka Became Karlachs' One and Only, but I can see that you haven't played the game since this post. Anyway, I referred to my situation as a bug because it felt so stupid that I've missed such a big chunk of content only because I decided to go North only after tiefling party is over. I mean, there is some sense, no party with Karlach = no hot nights, but if only Karlach stops making any hints that she likes me in future.. Ayy, that hurts and was confusing af.
I'll try to romance her "properly" in the future.. Idk when, because my next playthrough will be as Dark Urge and she most definitely won't like it.. And idk when would I play the BG3 again, as the first playthrough took few months of my life and I still need to get some rest from it.
