I would say that Astarion, Lae'zel, and Minthara have tremendous synergy when it comes to your general decision-making if you are ruthless and pragmatic, and/or also clever and deceptive. I do not think you have to be playing a morally evil character to achieve that grouping either - it can be done by making gray decisions and putting the needs of your companions first and foremost, before the wants of others.
I mean, don't get me wrong, I'd love me some good old bitey-bitey romance, but Astarion's personality is just insufferable to me, the sly and sleazy "I'm the god's gift to everyone" snobbish attitued of his, most of the time he annoys the Hells out of me :D
If you feel that way it's even more of a reason to do his companion quest I think. He's a very well written character with a really great arc.
I could write a whole essay about how they used vampirism as a allegory for generational trauma and cptsd but honestly you are better off seeing it for yourself! 10/10 redemption arc, no notes.
I saw someone say his approval is inconsistent because has a love hate relationship with heroes. He wants to believe in them but he also hates seeing people get help, because he's bitter about the fact that one ever helped him when he needed it.
