It IS annoying though, so if it bothers you, just google and use zanzer's cheat engine table to automatically give yourself +100 approval with Astarion and continue making good decisions he will disapprove of without much consequence.
Then astarion will be maxed out on rep with you
If you look at reddit there's some handy guides for specific approval encounters and how to trigger extra cut scenes (this one in particular hits a lot of points in the comments https://www.reddit.com/r/BaldursGate3/comments/q7fdhc/astarion_cutscenes/ )
It helps to have him in the number 2 of your party, as apparently party order helps determine whose cut scenes show when they trigger multiple at once. Hopefully they will implement an easier way to get these to show though as right now it feels like I'm playing 4D chess just to get him to tell me about a dream he had lmao
Minthara, Lea'zel and Astarion will approve of rough and inhuman decisions, they will revel in merciless actions. If you follow that path you will end up with other allies in the end as well, as there certainly are "bad" ones too. It does make not make a difference in supportive effort in the end battle though, although i much prefer Mizora over Lady Aylin.
The rest are the good guys approving of good deeds and merciful decision. Don't mix these characters if you want them all to improve in standing.
You can also split the party and have him sit just outside the spawn range. I do this with Karlach too when I am making questionable choices lol
Many cut scenes you just have to indicate you trust him. So when he wants to stand guard, say that you trust him. Bite scene, let him bite you. Next day, say you trust him when the party members start condemning him as a vampire. Pocket the ring from the tiefling kid when he is doing his trick. In the Putrid Bog (this is only if you see through the illusion of the sunlit wetlands) baa x 2 at the red caps. Tell Raphaels you would be insane to take a deal from the devil. At the barn, speak to who is inside then when trying to open the door, let Astarion open the door. Feed the owlbear cub at your camp. He also tends to approve of interactions where you attack first rather than try and hash things out with bag guys.
The biggest jumps in approval that I noticed are really the bite scene and the following day. My approval jumped up a pretty good chunk.
I technically wouldn't classify Astarion as evil. I find him more neutral oriented. He actually sometimes agrees with some good aligned options.
