Whether you use them is up to you. Some of your companions want them by default (Astarioon) and some are opposed by default (most others)
You can convince those that don't want to use them that they should, and if you succeed, they'll be open to it.
That's all. don't click the option if you don't intend to talk them into it.

Personally I also see it as an oversight that you can't tell Karlach or Gale that the tadpoles can probably save them from their conditions, like Astarian's.
But nobody's forcing you to persuade them to use the tadpoles. It's just you persuade them or you ignore it.
But there's no consequence for it yet until you get to a point where you have to make a second persuasion check later in the game.
This is such a disappointment. It seemed so integral to the story.
The decision to take tadpoles early doesn't matter much, there's a special option later where it starts to really matter.
