What do you expect in this short time, that they change here complete story line? Here current ending is sad and yes it would be much much better... like simply fix her heart! This is only one thing they CUT because it wasnt enough time to make it before release! The game isnt perfect and finished yet.....

They lied and called it a new ending but it's literally just a single dialogue check to prevent the bug where Wyll will go with Karlach if you've romanced her.

And most of them aren't full on happy either, Gale's the only one who seemingly has a 100% no drawbacks happy ending. Everyone else's happy ending has a catch.

"While Minthara's fixes won't be included in Patch 2, Vincke says fans can expect a new epilogue for fan-favorite Karlach in the upcoming update (he cryptically hints that if you see a cigar you'll know that you've found the new content), and that it will "make a lot of people happy." It will also include improvements to co-op as well as a "number of other things people have been asking about."

I haven't started a new playthrough, so I got no idea what up with any cigar. But if there is added some sort of new content to be found, I assume just loading up a old save isn't enough.

AFAIK Larian haven't yet said they will add a happy ending for Karalach or that if they were doing it, it would be in this patch. Maybe they will, maybe they won't. I'd love to see a happy ending for her though.

EDIT: Just read the previous post, so maybe they planned it, but its not triggering correctly, or people just haven't found the right conditions to make it trigger yet.

Literally none of that is in game, nor in the patch notes. To be clear for anyone who hasn't figured it out yet: Ruffio is making ♥♥♥♥ up and claiming it's in this patch when it isn't.
