Piecing together the half truths the Emperor tells you;
He undertook his second doomed mission to the other continent, which assuming they haven't retconned the shipwreck discovered in an earlier game, ran afoul from an outbreak of the werepuppers on the long return back. Somehow he was rescued from that fate, I'm presuming by his bronze dragon friend, but he couldn't face returning to his home city ashamed of a failed expedition, so instead he followed rumours of treasure underneath where Moonrise Towers would one day stand, and was instead captured by the Mind Flayers there and infected. Shortly after (I'm guessing 1-2 days, considering after a week he would be a Mind Flayer) he was rescued, again, by Ansur, who spent the next few days struggling to find a cure for him (I guess Auntie Ethel wasn't around 500-1,000 years ago, since she could for sure cure a standard tadpole infection). As more and more of his brain was consumed by the tadpole, the more and more he started to become the Mind Flayer but his personality was strong enough to remain intact and imprint itself on the nascent monster. Eventually Ansur tries to offer his friend a final mercy to spare his soul, so somehow a mind flayer that hasn't even yet fully grown and got ♥♥♥♥♥ slapped by some level 8 githyanki honor guards took down a level 17 bronze dragon and killed him.
What he does for the next few centuries/millenia, yet alone HOW he manages to stay alive when Mind Flayers only live for ~120 to 160 years, is a complete mystery, until Gortash barges in and reintroduces himself with the Elder Brain that Ketheric built a tower above. For some reason.
Only Quasi deities like Bane, Bhaal and Myrkul are an exception to that rule.
Bane in particular was a greater deity in 4E and likely in the top 3 of most powerful evil deities in Faerun but seemingly loved meddling so much that he forsake most of his power just so he can continue with Faerun affecting schemes.
And Helm is easily among Ao's most loyal followers sooo...
